Overview
Westervelt College requires your consent in order to collect your personal information so that we may comply with the above named Act. The following lists the variety of data that is required from you, the reasons as to which we require the information, and which individuals or organizations we will be sending the data to. All personal information is reserved in individual student files, that are locked in the Administrative office each evening, and will remain in said file for a period of 7 years or as long as the College remains in operation. Each evening a state-of-the-art alarm system is activated at Westervelt College in order to maintain security of the Building and all its components. The President and Administrative staff are the only individuals having access to personal student files, any other people having or requiring access will be determined below.

Director of the PIPEDA Act
Wendy Teetzel, Campus Administrator is the Director of the Act. Should for any reason she be absent from her position, an appropriate Westervelt administrative staff member will be the Interim Director. Upon commencement of employment at Westervelt College, each staff member is required to sign a Confidentiality Agreement. This agreement asserts that an Employee must at all times protect and treat any information about a student or the organization as Confidential, and all information can and will only be shared with the individuals/organizations that are outlined in the Personal Information Protection and Electronic Documents Act agreement.

Name, Address and Telephone Number
These are required so that we may register you for your program or course, for any correspondence needed between the student and the College, and for contact for various reasons at any time. This information is shared with the Financial Office so that the Administrator may process your T2202 tax forms and observe your payments to the college. Your name, address, and telephone number may also be disclosed to a potential employer upon completion of your program, your Funding Organization/Worker, and any administrative staff member.

Acceptance
The use of the website and its forms may constitute implied consent for Westervelt College to collect, use and disclose personal information for the identified purposes.

Birthdate, Registration, photo ID, Secondary School Diploma, Entrance Test
Registration by the Ministry of Training, Colleges and Universities (MTCU) requires this information in order to prove you are 18 years of age or older and have met the qualification requirements in order to participate in the registered course. Again, all information may be shared with your Funding Organization/Worker and any administrative staff member.

Social Insurance Number
Your Social Insurance Number confirms that you are a Canadian citizen and/or landed immigrant; therefore, this information may be required to ensure that you are eligible for the program. Organizations that may also require this information are Human Resources Development Canada (HRSDC) and the Work Safety Insurance Board (WSIB) if you are being funded through them. This information may be sent or faxed to them at any time, usually a receipt of payment towards the College is sent with this information. If you are being funded by OSAP or have amounts owing to OSAP your SIN will also be sent to them for Schedule 2 purposes. Any administrative staff will also have access to this record.

Name and Phone Number of Emergency Contact
Should for any reason you become ill or acquire an injury to yourself, this information is kept so that we may contact someone to assist you. This information is also helpful when an administrative staff member cannot speak with you by normal procedures.

Financial Payment
The Financial Office keeps all financial transactions. When payment is received, receipts are filed in secure individual student files held in the administrative office. This information assists in monitoring your payment schedule predetermined with the College and to produce your T2202 for income tax purposes. This information could be shared with your Funding Organization/Worker or Financial Administrator.

Copy of Diploma/Certifications and Transcripts
A copy of these documents are filed in individual student files in case for any reason you require another copy, should an employer have need of a reference/verification of completion, and/or your Funding Organization/Worker or administrative staff member needs to view your documents.

Resume
Graduate Employment Services requires your resume in order to assist you with your search for employment upon completion of your program. A copy of your resume may be sent to a potential employer or work placement organization.

Medical and Criminal Record Checks
A Medical Note from your doctor and a Criminal Check from your local Police Service are a requirement for enrolment in, but not limited to, Pharmacy Assistant, Personal Support Worker and Police Foundations programs. These programs require this information to ensure you are physically able to work and participate in the Health Field or Police Services environment. Should any of Westervelt College’s courses/programs involve participation from the National Association of Career Colleges, the Ontario Association of Career Colleges, and/or the Ministry of Training, Colleges and Universities, then any or all of the above mentioned information may be viewed at any time.
